- Your national ID copy showing both sides passport or birth certificate
- A KNEC bank deposit slip of Kenyan shillings 5000 per certificate replacement this should be paid at KNEC collection accounts in any branch of the following banks

- Co-operative bank of Kenya
- National bank of Kenya
- Kenya commercial bank, all the KNEC deposit slips are issued in these banks.

Download lost certificate application form from KNEC website and fill in carefully, on the application the following items are mandatory
- Name of applicant
- School and examination center
- Full index number
- Address and contact information
- Name of the examination KCPE, KCSE,KCE,CPE
- National Id
- Year of examination
- Applicant signature
- Proof of payment
After filling this form correctly with all the details, submit the form attached with all documents to KNEC council secretary or chief executive using courier service or postal offices

The certificates are usually ready for collection after sixty working days from KNEC offices. The certificate should be picked in person on presentation of ID. Upon 2 years without collection the certificates are destroyed without communication and no refund on money paid.
